need help integrating custom form

so i'm at the stage of customizing the forms. great so far, but i found a custom form style i want to copy. however, it's hard for me to figure out how to integrate the rails into its style since it is much more complicated than the example used in the tutorial. i would greatly appreciate some guidance on how to create my form in this style: http://bootsnipp.com/snippets/featured/long-inline-form

Everything between these two tags is what matters <form></form>
delete all the inputs you don't need first to narrow it down and clean up the form do you need all those inputs this is a very text heavy form because of all the select inputs. I would start out with a more basic form and get the hanging of deleting out inputs you don't need adding in inputs you do need and using rails form helpers...this will be for registration right? if so your dealing with a model backed form.

There isn't a lot of material on how to do this yet but i think in the near future Alex should do a screencast on it because it would be better explained on how to take premade html forms and integrate them into your rails app using rails helpers for model backed or non model backed forms.

ya.. everything related to forms in the mvc framework is pretty much what rails is about, from routes to actions and model objects forms and how we handle them is rails. So it is a deep subject for sure and does need a lot of understanding first to really play around with.
